San Diego’s Newest Street Outreach Service Provider

STREET OUTREACH & EDUCATION

The SoCalYES Street Outreach team goes out and finds Homeless, Run Away & At-Risk Youth who may need help in San Diego County. The team provides:

  • Immediate transportation to Emergency Shelter and
    assistance with the initial check-in process.
  • Food & hygiene kits, water, warm clothing, blankets,
    backpacks, shoes, sleeping bags to every young person
    encountered for their immediate needs.
  • Information and referrals for Housing, Medical & Dental,
    Substance Abuse Rehabilitation Programs, Behavioral
    Health & Counseling Services, HIV/STD Testing &
    Prevention, Education Services, Food Assistance Programs
    and much more.

The SoCalYES team is a staff of culturally sensitive individuals trained to provide quality interactions and care for youth who identify as LGBTQIA+. We are well aware that without the ability to collaborate and partner with culturally competent providers, youth may be less likely to seek much-needed services.

The SoCalYES Street Outreach Team’s services are always free, voluntary and nonjudgmental. Unless otherwise defined within the text, when we use the terms “youth”, “young adults,” or “young people” we are specifically referring to young people between the ages of 12 and 24.

CRISIS INTERVENTION

SoCalYES Street Outreach Team provides crisis intervention services to youth, when a youth accepts our services, or there is a critical need placing youth at risk for running away or becoming homeless. We work with emergency shelter staff for intake and assessment.

Prevention is the critical first step toward an effective response to youth homelessness. Not all incidents of youth homelessness can be prevented, but with appropriate, targeted street outreach services, our team will immediately work with every young person encountered to help them access immediately available services so that each young person can avoid crisis.

24-HOUR CRISIS HOTLINE
– “YES LIFELINE”

Contact the YES Lifeline 24-hours a day toll free at 1-833-SOCALYES (1-833-762-2593) for support, information, and referrals.

The YES Lifeline is available for youth, parents, family members, and concerned citizens. Our Hotline is available 24/7/365 to any youth encountering homelessness or at-risk of encountering homelessness can call anytime day or night and talk to a live person to discuss immediate help.
The SoCalYES Street Outreach Team gives every young person they encounter a hard-plastic YES Lifeline Emergency Hotline card.

BASIC SURVIVAL TOOLS (CLOTHING/FOOD/HYGIENE)

Every SoCalYES Street Outreach vehicle is always fully equipped with basic survival aid in the form of clothing items, blankets, sleeping bags, backpacks and food & hygiene kits. When one of our outreach workers comes across any youth in need, they are ready with the basics for these youth, taking care of their immediate needs.

INDIVIDUAL ASSESSMENTS

SoCalYES works directly with Drop In Centers & Youth Shelters in San Diego County that provide a safe refuge from the streets. For some young people, these are the only indoor spaces they can access. In addition to shelter, services include:

  • Hot meals
  • Showers
  • Laundry services
  • Computer access
  • Behavioral health services

These entry-level services give young people the support they need to formulate a plan.

INFORMATION & REFERRALS

Our SoCalYES Street Outreach Team offers Information & Referrals and brochures to every young person they encounter. In San Diego County there are an abundance of services available to them, catering to their individual needs. Depending on an person’s goals, our team is able to provide proper information and direction to help them get the services they would like or need.these services directly involve case managers that provide Individual Assessments and a case plan, for each and every young person, based on their individual needs.

TRANSPORTATION ASSISTANCE

SoCalYES offers transportation at no cost for youth seeking services from anywhere in San Diego County to approved youth serving agencies such as:

  • Shelter
  • Medical/Dental
  • Counseling
  • Rehabilitation Services

FAMILY REUNIFICATION SERVICES

The SoCalYES team always works to reunify youth, who have run away from home. Whenever family conflict can be safely remedied we seek reunification. Otherwise we seek to identify safe living alternatives for youth when family reunification is not possible or is in the best interest of the youth or family.

FOLLOW-UP SUPPORT

SoCalYES offers follow-up support to all youth that accept services through our program. There are many ways in which we provide follow-up support, including:

  • Carrying out home visits to youth who have been reunited with family, but are at significant risk of homelessness
  • Helping to advocate for youth, that have accepted services with other youth-serving agencies, to procure additional services
  • Enabling clients to establish and maintain links with appropriate services, including services relating to general health
  • Offering ID and document replacement support. In order to obtain additional services, youth need the necessary ID’s & Documents
